The Process
1. Book Appointment
If seeking insurance coverage, please verify benefits before booking an appointment. As a new patient, you can choose the option to book a new patient appointment.
2. Fill Out Paperwork
You will be requested to fill out important intake forms on our EMR portal prior to your appointment.
3. Nutrition Visit
Sessions are conducted in person or via telehealth on our HIPAA compliant portal. You will be sent a link to access our meeting if it's via telehealth. This allows for greater convenience and flexibility with our patients.
4. Develop Plan
We will then formulate a specific and realistic plan to help you address your health needs. This may include recommendations for diet, supplements, lab testing, and other lifestyle recommendations. Then the progress happens!
